The aesthetic medical industry has been booming year on year for the past 25 years and shows no sign of abating.  You can be forgiven for being confused by all the treatments now on offer, all promising youth and beauty.

I’ve always maintained that, as far as non-surgical treatments go, Botox®, dermal fillers and medical grade skincare offer all we need to ensure that the very best results are achieved.  I do research and consider all the ultrasound/radio frequency treatments on offer.  New high street clinics are sprouting everywhere.  They buy expensive machines and use marketing ploys to promise great results. Unfortunately, I just cannot see how the results can be anything more than “debatable.”  This is because the effects are primarily on the skin’s texture, not on its structural support.  Therefore, photographically significant lift, in my view, cannot be achieved.

Results from these treatments are not immediate (unlike Botox® and dermal filler treatments).  Clients tend to be advised that the results are best after waiting for 6 months to pass. Frankly, after 6 months the client is likely to have forgotten not only what they looked like before treatment but also the large debit from their bank account for the privilege.

Botox® erases all the worry lines/wrinkles caused by the way we express ourselves and continues to be my number 1 treatment for all age groups to freshen and rejuvenate the skin.

Fillers, despite the bad press received, have become more and more important as we understand that the effects of ageing are primarily due to volume loss. Strategic use of fillers in skilled hands gives you an “invisible” face  lift.  Fillers can make you look 10 years younger in a totally natural way.

However, fillers are not only used to correct volume.  An aesthetic doctor with an eye for facial beauty can upgrade your appearance with skilfiul use of small quantities of filler.  Leonardo Da Vinci demonstrated the ”Phi” golden ratio in that the ideal face conforms to strict proportions.  By way of illustration, the faces of photogenically beautiful women, eg. Angelina Jolie, Kate Moss, all conform to the “Phi” golden ratio.  These relative facial dimensions are multiple and include width of cheeks relative to overall length, eye position relative to nose length and width, mouth size, chin width and length, etc.  I find myself routinely asking myself what I can do to make my clients more attractive by subtle placement of fillers.  Small adjustments can make a big difference and the effects can last for years.

I’ve been using Obagi skincare supplemented by tretinoin cream myself for 22 years.  There are so many skincare ranges available now that you’d be forgiven for being confused by all the ingredients mentioned and by the fact that they all seem to promise smoother, less wrinkled skin.

As any dermatologist will tell you, moisturisers will not remove or prevent wrinkles although they will give your skin the appearance of being smoother. They  will alleviate the dryness initially experienced by tretinoin.  This dryness is an essential step in the process as the dead skin cells are exfoliated off to reveal new skin cells beneath.

What is important and scientifically proven in medical, double blinded trials is the addition of prescription products to your daily regime.  These are only available by doctors/prescribing nurses. The most important addition is medical strength retinol cream at night, twice or more per week. The strength necessary is tretinoin 0.05% (or retinol 2.0 equivalent concentration).

Tretinoin 0.05% promotes skin turnover meaning that you will slough off the dead skin cell layer, revealing fresh, beautiful skin.  It will also stimulate the production of collagen in the deeper skin layer.  This is the most important advantage of this product.

The other product which, although not a prescription item, will make a difference to your skin over the years is a good quality vitamin C serum. Vitamin C is an antioxidant and therefore protects and repairs your skin from the effects of the environment.  I use Obagi’s Vitamin C Serum 15% every morning and find that its moisturising quality combats any dryness from tretinoin the night before.

A high SPF sunblock should be used regularly.  Rates of malignant melanoma, one of the deadliest of cancers, are on the rise.  Finally, for those of you who suffer from melasma, Obagi’s hydroquinone containing products (Clear and Blender) are industry leaders.  Do book a skincare consultation with me and I’ll cherry pick the most important products for your skin.

With increasing publicity of this product over the years and celebrities endorsing its lash lengthening/thickening effects, this treatment has become incredibly popular.  Many report benefits on brow hair as well.  Please note that this product has been scientifically proved to work in controlled medical trials.  It is prescription only and you will be required to approve a consent form before using it.

This product needs to be used daily for the first three or four months (after which the frequency can be reduced).  When you start using this product, a 3ml bottle will last one month and no more.  Of course, once your lashes have grown, it is likely that maintenance is obtained by twice weekly use (in which case each bottle might last you several months).
